Teams from across the health and care system in Lincolnshire are a quarter of the way through their 100 Day Challenge to improve outcomes for people living with frailty.

100 Day Challenges are about rapidly testing ideas to see what works and what doesn't work, with ideas evolving and new ideas emerging over the course of the challenge.

The teams are working on four specific areas: reducing frequent A&E attendance (Gainsborough), frailty & homelessness (Lincoln North), mental health & people living alone (Mablethorpe), and falls prevention (Stamford).

The challenge has now passed its first milestone, day 25, and the teams recently came together to review progress.

The teams have been working hard over the first 25 days to build connections, identify people living with frailty, and lay the groundwork for their ideas which they are starting to test and will continue to do so over the next 25 days.

These ideas include the development of a train-the-trainer curriculum on frailty, the creation of a multi-disciplinary team hub with more efficient ways of communicating, adapting tools for identification and even exploring providing access for pets in shelters.  

Importantly, this challenge is also about finding new ways of working and strengthening collaboration.

Teams were already reporting what a huge difference it has made to feel connected to other practitioners in their neighbourhood, and to know that they are part of a strong network of passionate frontline staff all working towards the same goal of improving outcomes for people living with frailty.
