Out Of Hospital Care
We are committed to achieving a significant change in the delivery of out of hospital services in order to keep people closer to home safely for longer. We work daily with social care and voluntary organisations, as well as Lincolnshire Community Health Services NHS Trust (which is rated outstanding by the CQC) to ensure our plans are deliverable.
Our programme aims to accelerate change in the way we look after you, your family or loved ones in the community with particular emphasis on two strands:
Align and define
- Align and engage local leaders to set a high level of service and delivery in the community, with new ways of collaborating and working together
Population health management
- Understanding what the population needs
- Understanding how people use our services
The scope of this programme will include the following services:
- Primary care (general practice and community pharmacy), community health services, social care, domiciliary care (home care), mental health and the third sector - charities and voluntary groups
